I will not attend this event if I or anyone I am booking tickets for :
o has been diagnosed with COVID-19 in the previous 7 days.

o has been in close contact with an active COVID-19 case in the previous 7 days

o has been to a major outbreak venue or super spreader event reported in Queensland.

o had a fever, cough, sore throat, headache, distorted sense of taste, shortness of breath, chills, vomiting, or any cold/flu-like symptoms in the last 72 hours.

I agree that I will inform Emmanuel Uniting Church if I or anyone I have booked tickets for, becomes a confirmed or suspected case of COVID-19 or recognised as a close contact.

When I attend this event, I will adhere to all safety, hand washing, and physical distancing measured as outlined in the guidelines and as instructed at the event.
